Okay the form is a bit confusing but hope this works for you all:

Enter in DOB
DD MM YYYY instead of month first and day second (MM DD YYYY)
day month year
Ex: 27 12 2004

Next: Postal Code you enter your STATE, ZIPCODE

EX: AZ, 85355
